抑郁与慢性疼痛共病的研究进展

Research progress on the common pathologies of depression and chronic pain

摘要:

抑郁症是全球最常见精神类疾病之一,其发病因素多种多样,但疼痛的长期存在是其中一个重要因素。慢性痛是一种长期存在迁延不愈的身体不适感,往往会导致多种并发症,其中抑郁是最常见的并发症之一。疼痛与抑郁往往互为因果,共同出现,其共病机制复杂,目前尚未完全阐明。众所周知的是,海马体是调节慢性疼痛和抑郁症神经通路和神经递质的共同区域,是疼痛和情绪障碍产生的可能联系。此外,趋化因子家族在神经炎症过程中扮演着重要的角色,同样也参与了抑郁情绪产生的关键过程。本文从近年来疼痛与抑郁之间可能存在的共同病理联系进行总结,以期为慢性痛伴发的抑郁情绪障碍提供新的治疗思路。

Abstract:

Depression is a common mental disorder worldwide, and many pathological factors could contribute to its development. Among that, the most significant factor is the persistent presence of pain. Furthermore, chronic pain refers to long-lasting physical discomfort which always closely associated with depression. However, the potential pathogenesis between chronic pain and depression is complicated and remains to further demonstrated. As we all know, the hippocampus plays a key role in regulating chronic pain and depression because of the common neuron pathways and neurotransmitters, which maybe is the potential connected of chronic pain and depression. Additionally, the chemokine family members may contribute to neuroinflammatory processes as well as the depression development. This article summarizes the recently researches on the possible underlying pathological mechanism between the chronic pain and depression, and aims to provide a new perspectives for the chronic pain along with depression-like negative emotions treatment.
