星状神经节阻滞联合度洛西汀治疗交感神经型颈椎病

Clinical observation on treating sympathetic cervical spondylosis by stellate ganglion block combined with Dutoxetine

【摘要】 目的:通过观察星状神经节阻滞联合度洛西汀治疗交感神经型颈椎病患者的临床疗效，以期为交感神经型颈椎病的治疗提供新的理论依据。方法:将90例临床诊断为交感神经型颈椎病的患者随机分为治疗组、对照组和安慰剂组，每组30例，治疗组采用星状神经节阻滞联合度洛西汀治疗，对照组单纯采用星状神经节阻滞治疗，安慰剂组以星状神经节阻滞+口服淀粉片治疗，以8周为1疗程，评价治疗前及8周治疗结束时及治疗后4周三组患者VAS评分及SF-36评分、焦虑自评量表( SAS) 和抑郁自评量表( SDS) 及睡眠质量评分(PSQI)等变化情况。结果:经8周治疗后，三组患者VAS评分及SF-36评分、焦虑自评量表( SAS) 和抑郁自评量表( SDS) 及睡眠质量评分(PSQI)较治疗前明显改善，且治疗组优于对照组和安慰剂组(P<0.05)，差异具有统计学意义。结论:星状神经节阻滞联合度洛西汀口服在改善交感神经型颈椎病患者疼痛、生活质量及焦虑、抑郁等方面优于单纯星状神经节阻滞治疗，且无明显不良反应，值得临床推广应用。

Abstract:

Objective: To observe the clinical effect of stellate ganglion block (SGB) combined with Dutoxetine in treating sympathetic cervical spondylosis. Then provide theoretical basis for the treatment of sympathetic cervical spondylosis. Method: Select 90 cases of sympathetic cervical spondylosis patients. And divided them into treatment group and control group and placebo group randomly.. 30 cases in every group .Treatment group was treated by SGB combined with taking Dutoxetine by mouth. while the control group was treated by SGB only. Placebo group was treated by SGB combined with taking starchtablet by mouth.8 weeks is a course of treatment. Then appraise the transmutation of VSA scale、SF-36 scale、self-rating anxiety scale（SAS）、self-rating depressive scale（SDS）and Pittsburgh sleep quality index number scale（PSQI）before and after treatment. Result: After treatment of 8 weeks，compared with pretherapy，the VSA scale、SF-36 scale、self-rating anxiety scale（SAS）、self-rating depressive scale（SDS）and Pittsburgh sleep quality index number scale（PSQI）in two groups are all improved.The treatment group is better than control group and placebo group ( P<0.05). Possess the statistical significance. Conclusion: In relieving pain、raising quality of life、and relieveing anxiety-depression of sympathetic cervical spondylosis patients,the SGB combined with Dutoxetine are better than the control group,have no obvious adverse reaction，and can be popularized in hospitals of various levels.
