脉冲射频联合连续射频热凝治疗胸段带状疱疹后神经痛的疗效与安全性研究

Study on the efficacy and safety of pulsed radiofrequency combined with continuous radiofrequency the treatment of thoracic postherpetic neuralgia(PHN)

摘要:

目的：评估脉冲射频联合低温(45℃)连续射频治疗带状疱疹病后神经痛疗效及安全性。方法：选择2020年3月~2021年7月在我院疼痛科住院治疗的胸段(T1~T12)PHN患者120例,随机将患者纳入观察组(PRF联合低温CRF治疗)和对照组(PRF治疗),每组60例。采用简SF-MPQ评估患者疼痛改善情况；采用PSQI评估患者睡眠改善,并评估治疗的并发症和不良反应。结果：治疗后3d,对照组和观察组患者疼痛、睡眠明显好于治疗前,差异有统计学意义(p=0.00),但两组SF-MPQ总分和PSQI评分无差异(p>0.05)；治疗后1W至24W对照组患者SF-MPQ总分和PSQI评分逐渐回到治疗前水平,观察组患者SF-MPQ总分和PSQI评分持续降低,并显著低于对照组(p<0.05)和前一个检测时点(p=0.00)；两组患者治疗后并发症和不良反应均少见。结论：脉冲射频联合低温连续射频治疗可显著减轻胸段PHN患者疼痛和睡眠,且持续时间长,并发症和不良反应少。

Abstract:

Objective: To evaluate the efficacy and safety of pulsed radiofrequency(PRF) combined with low temperature (45℃) continuous radiofrequency(CRF) in the treatment of thoracic PHN. Methods: 120 thoracic (T1~T12) PHN patients hospitalized in the pain management department of our hospital from March 2020 to July 2021 were selected. The patients were randomly divided into observation group (PRF combined with low temperature CRF treatment) and control group (PRF Treatment), 60 patients in each group. SF-MPQ was used to assess the pain relief of patients; PSQI was used to evaluate the improvement of patients’ sleep, and to calculate the complications and adverse reactions. Results: 3 days after treatment, pain and sleep quality of patients in both control and observation group improved than before, the difference was statistically significant (p = 0.00), yet total score of SF-MPQ and PSQI score were not different between the groups (p> 0.05); from 1W to 24W after treatment, the SF-MPQ total score and PSQI score of the patients in control group gradually returned to the pre-treatment level, while the total SF-MPQ score and PSQI score of whose in the observation group continued to decrease, and were significantly lower than those in the control group (p <0.05) and previous detection-time (p=0.00); Conclusion: PRF combined with low-temperature CRF can significantly relieve the pain and improve sleep quality of patients with thoracic PHN, and the efficacy have relative long duration and few complications and adverse reactions.
