肩胛上神经阻滞联合经皮穴位电刺激 治疗肩周炎临床效果

ClINICAL EFFECT OF SCAPULAR NERVE BLOCK COMBINED WITH TEAS TN THE TREATMENT OF SCAPULOHUMERAL PERIARTHRITIS

摘要:

目的 评价肩胛上神经阻滞联合经皮穴位电刺激法治疗肩周炎的临床效果。 方法 90例肩周炎患者，年龄18-65岁，性别不限，采用随机数字表法分为神经阻滞组（A组）、神经阻滞联合真穴位刺激组（B组）、神经阻滞联合安慰剂穴位刺激组（S组）。A组进行肩胛上神经阻滞，B组同时进行肩胛上神经阻滞与真穴位电刺激（电流强度15-25 mA），S组进行肩胛上神经阻滞与安慰剂穴位电刺激（电流强度5mA），隔天治疗一次，六次为一个疗程，共两个疗程。观察记录各组患者治疗前（T0）、治疗第1天（T1）、治疗第3天（T2）、治疗第5天（T3）、治疗第7天（T4）、治疗第9天（T5）、治疗第11天（T6）、第二疗程末（T7）的疼痛感觉以及肩关节功能变化情况，并比较两个疗程结束后患者的满意度。结果 与A组比较，S组患者在各时间点的疼痛VAS评分与肩关节功能评分差异无统计学意义（P＞0.05）；B组患者在各时间点的疼痛VAS评分均较低、肩关节功能评分均较高（P＜0.05）。与S组比较，B组患者在各时间点的疼痛VAS评分均较低、肩关节功能评分均较高（P＜0.05）。两个疗程结束后，B组患者的满意度较A组与S组均较高（P＜0.05）。结论 肩胛上神经阻滞联合经皮穴位电刺激治疗肩周炎可产生更好的效果且易被患者接受。

Abstract:

Objective To evaluate the Clinical effect of scapular nerve block com- bined with TEAS in the treatment of scapulohumeral periarthritis. Methods Nine- ty cases of scapulohumeral periarthritis, aged 18-65 years old, gender not limited，we- re divided into 3 groups（n=30）using a random number table：scapular nerve block group（group A）,scapular nerve block plus TEAS group（group B）,scapular nerve block plus sham TEAS group（group S）.Subscapular nerve block was performed in group A.Subscapular nerve block and true TEAS were performed simultaneously in group B.Subscapular nerve block and sham TEAS were performed simultaneously in group S.Treatment is once every 2 days, 6 times for a course of treatment, a total of two courses.Each groups' sense of pain and the change of the shoulder joint function of ASES were observed and recorded before treatment (T0),1 day after the treatment (T1), 3 days after the treatment (T2), 5 days after the treatment (T3), 7 days after the treatment (T4), 9 days after the treatment (T5), 11 days after the treatment (T6), at the end of the second course (T7) , and patient's satisfaction was compared at the end of two courses treatment. Results Compared with group A,the VAS of pain were de- creased and the shoulder joint function of ASES were increased in group B,and no significant change was found in the parameters mentioned above in group S.ComPar- ed with group S,the VAS of pain were decreased and the shoulder joint function of ASES were increased in group B.At the end of two courses treatment,patient's satis- faction in group B was higher than that in group A and group S. Conclusion Sca- pular nerve block combined with TEAS in the treatment of scapulohumeral peri- arthritis has better treatment effects and can easily be accepted by patients.
