先兆偏头痛合并卵圆孔未闭行封堵治疗的meta分析

META-ANALYSIS OF PERCUTANEOUS PATENT FORAMEN OVALE CLOSURE FOR TREATMENT OF MIGRAINE WITH AURA

摘要:

Meta分析在先兆偏头痛（MA+）合并卵圆孔未闭（PFO）的患者中行封堵治疗的疗效。方法 检索国内外多个电子数据库，纳入在MA+患者中行PFO封堵治疗与药物治疗的随机对照试验（RCT）和队列研究，采用Review Manager 5.3对数据进行分析。结果 共纳入5项研究，其中3项RCT和2项队列研究，入组362名患者。Meta分析显示，以每月偏头痛发作减少≥50%为主要结局指标，封堵组比药物组效果好[OR = 4.29，95% CI( 2.67，6.97)，P ＜0. 00001]。在减少每月偏头痛发作天数上，封堵组优于药物组［MD = -1.62，95% CI( -3.01，-0.24)，P =0. 02］结论 在MA+合并PFO的患者中行封堵治疗，对减少偏头痛的发作有一定的疗效。

Abstract:

To systematically analyze whether there is benefits of patent foramen ovale(PFO) closure in patients with migraine with aura(MA+). Methods Papers on the effect of PFO closure of migraine and/or MA+ in randomized controlled trials and cohort studies were retrieved from a number of foreign and domestic database.The data was analyzed by Review Manager 5.3. Results Three randomized controlled trials and two cohort studies were included in this analysis, including 362 patients. Meta-analysis showed, reduction in monthly migraine attacks/days ≥50% was higher in PFO closure compared with the control group[OR = 4.29，95% CI( 2.67，6.97)，P ＜0. 00001].There was higher reduction of monthly migraine days in PFO closure group compared with control group［MD = -1.62，95% CI( -3.01，-0.24)，P =0. 02］.Conclusion PFO closure might be beneficial in migraine with aura patients by reducing migraine attacks and migraine days.
