微针透皮给药治疗带状疱疹急性痛的效果观察

Observation on the effect of transdermal inject of microneedle in the treatment of acute herpetic pain.

【摘要】 目的 观察微针透皮给药治疗带状疱疹急性痛的效果。 方法 选择门诊急性带状疱疹痛患者45例，年龄在27~66岁，随机均分为常规药物组（A组），口服激素组（D组）和微针治疗组（W组）。A组口服阿昔洛韦+曲马多+弥可保；D组口服阿昔洛韦+曲马多+弥可保+地塞米松片，阿昔洛韦+曲马多+弥可保口服方法同A组,地塞米松片0.75mgx3片BID口服3天；W组地塞米松注射液2mg+0.5%利多卡因共20ml，加生理盐水稀释至所需药量，沿病变边缘定点，两点相距1.0cm,在两点间隔中进行微针注射，每次注入1~1.5ml治疗液，两个注射点之间间隔1cm，至需要阻滞的范围阻滞完，药物口服同A组。记录患者治疗前（0天）的VAS评分，及皮损面积（%）及通过电话随访分别记录患者治疗后1天（1天）、治疗后2天（2天）、治疗后3天（3天）、治疗后1周（7天）和治疗后2周（14天）的VAS评分，以及治疗后皮损愈合时间（天）和疾病愈合时间（天）。 结果 比较三组患者治疗后1天，治疗后2天，治疗后3天，治疗后7天和14天的VAS评分，D组和W组的VAS评分均明显低于A组（P<0.001）;两组的皮损愈合时间和疾病愈合时间也明显短于A组（P<0.001）。但是，W组治疗后一天、二天的VAS评分低于D组（P<0.05）。 结论 微针透皮给药皮内注射+药物口服治疗能明显改善带状疱疹急性痛并能加速皮损的愈合和缩短病程。

Abstract:

【Absatract】Objective To observe the effect of transdermal inject of microneedle in the treatment of acute herpetic pain. Methods Forty-five acute herpes pain patients in outpatient clinic, aged 27~66 years, were randomly divided into 3 groups: the group routine drug treatment(group A,n=15),the group oral hormone treatment(group D,n=15)and the group microneedle injection+drug treatment(group W,n=15).The patients in the group A received the oral medicine(Acyclovior+ Tramadol +Methycobal). The patients in the group D received acyclovir+tramado+micobao+dexamethasone orally,dexamethame(0.75mgx3 tablets BID) for 3 days, acyclovir+tramado+micobao by the way of the Group A ;The patients in the group W received microinjection therapy with dexamethasone 2mg and 0.5% liduocaine 20ml to the dose required by increasing 0.9%Nacl,and were pointed along the end of the skin lesions,1cm between two points, injected with microneedle at two points with treatment solution 1~1.5ml,and received the oral medicine(Acyclovior+Tramadol +Methycobal);Recorded the skin lesion area of the patients (%) and the healing time of skin lesion(day) and theVAS scores of the patients :before treatment(0day), after treatment1day(1day),after treatment 3 day(3days),after treatment 1 week(7days),after treatment 2 week(14days).Results Compared respectively with the after treatment VAS scores of the patients in 3 groups,the VAS scores of the group W and the group D were lower significantly than the group A(P<0.001),compared with the healing time of skin lesion and the time of the disease healing of the parients in 3 groups,the time of the group W and the group D were shorter than the group A(P<0.001). But the VAS scores of the after treatment1day and 2 days in the group W were shorter than the group A(P<0.05).Conclusion Microacupuncture transdermal injection and oral administration of drugs can significantly improve the acute pain of herpes zoster and accelerate the healing of skin lesions and shorten the course of disease.
