硬膜外前侧间隙胶原酶溶解术治疗颈椎间盘突出症

PERCUTANEOUS ANTERIOR EPIDURAL CLEFT COLLAGENASE DISCOLYSIS IN THE TREATMENT OF CERVICAL DISC HERNIATION

摘要:

目的：评价经硬膜外前侧间隙胶原酶溶解术治疗颈椎间盘突出症的临床应用价值，并介绍此技术的操作方法及适应证和禁忌证。方法：采用硬膜外穿刺置管技术，取最低突出节段棘突间为穿刺点，正入将导管推向硬膜外侧壁，斜行置入硬膜外前侧间隙，经造影确定后，注胶原酶1800单位，观察胶原酶溶解术后的结果。结果：763例颈椎间盘突出症行此技术治疗后随访3个月～5年，优良率90．56％，有效率97．38％。结论：对诊断明确，有适应证的患者，在C型臂X线机的监视下，行硬膜外前侧间隙胶原酶溶解术，能解除神经根的压迫，具有安全、效果好、治疗时间短、痛苦小、操作较简单等优点，是治疗颈椎间盘突出症的良好方法之一，值得推广和进一步研究。

Abstract:

Objective:To evaluate the clinical application of collagenase discolysis in the treatment of cervical disc herniation. Method: Epidural catheter were placed at the anterior epidural cleft. Collagenase of 1800u was injected through the catheter. Results: The follow-up period was from 3 months to 5 years after the operation in 763 patients with cervical disc herniation. The rate of excellence and good was 90.56%. The rate of effectiveness was 97.38%. Conclusion: Under the monitoring of C-arm X-ray,patients with definite diagnosis and indications of the operation could be treated with this technique.
