HANS仪＋安定分娩镇痛的临床研究

HANS COMBINED WITH DIAZEPAM FOR LABOR PAIN CONTROL

摘要:

目的:应用韩氏穴位神经刺激仪（HANS）伍用安定,观察对产痛的缓解作用和对母婴的影响。方法:1998年12月～2000年2月期间,120例正常足月、单胎、头位、初产妇在宫口开大2～3cm时随机采用下列镇痛措施,包括HANS仪 安定（D）、单纯HANS仪和单纯安定（D）,与对照组比较产痛的VAS评分及对母婴各项指标的影响。结果:镇痛前,4组的VAS评分无统计学差异。镇痛措施后,HANS D组和HANS组VAS评分在镇痛后1小时、Ⅰ程末期及Ⅱ程时分别比镇痛前非常显著地下降（P<0.01）,与对照组相应各时期比较,有非常显著的统计学差异（P<0.01）。安定组VAS评分在镇痛后1小时比镇痛前及对照组非常显著下降（P<0.01）。三组相比,HANS D组VAS评分显著低于HANS组和D组（P<0.05）,单纯HANS组又显著低于D组（P<0.05）。结论:HANS仪结合安定进行分娩镇痛,使产程疼痛显著降低,而不影响分娩生理。但镇痛不全仍是一个有待改进的问题。

Abstract:

Objective: To evaluate the effect of combination of HANS (Han's Acupoint Nerve Stimulator) and diazepam (D)on labor pain. Methods; From December 1998 to February 2000, 120 normal term primipara with single vertex presentation were observed. They were randomly divided into four groups: HANS D (HANS plus diazepam 10mg,iv) , HANS, D (diazepam 10mg, iv) and control group. Pain relief measures were given when the cervix was dilated to 2 ~ 3cm. VAS (visual analogue scale) was used for evaluating pain degree. Safety of mother and the newborn was also monitored. Results: The VAS in HANS D group and HANS group was significantly lower than that of control groups (P <0. 01 ) . The VAS of group D one hour after analgesia was significantly lower than that of the control group ( P < 0. 05 ). At the time cervix dilation to 7 ~ 8cm and at the second stage, the VAS of HANS D group were significantly lower than that of HANS group and group D (P <0. 05) , and the VAS of HANS group was significantly lower than that of group D (P <0.05). Conclusion: HANS diazepam is a safe, convenient and effective method for the pain relief during labor, although the analgesia was only partial rather than complete.
