硬膜外注射维生素B_(12)缓解前列腺摘除术后膀胱痉挛性疼痛的临床观察

CLINICAL OBSERVATION OF EPIDURAL INJECTION OF VITAMIN B_12 FOR RELIEF OF BLADDER SPASTIC PAIN AFTER PROSTATECTOMY

摘要:

目的:评价维生素B12 硬膜外注射对耻骨上经膀胱前列腺摘除术后膀胱痉挛性疼痛的缓解效应。方法:选硬膜外麻醉下行耻骨上经膀胱前列腺摘除术患者60 例,随机分为三组,每组20 例,于硬膜外末次给药后45 分钟分别注入单纯生理盐水10ml(A 组) ,VB12 1mg 加生理盐水至10ml(B 组) ,吗啡2mg 加生理盐水至10ml(C组) ,以双盲法观察术后72 小时内各组膀胱痉挛性疼痛的发生情况。结果B、C 组术后膀胱痉挛性疼痛缓解优良率分别为85 % 、95 % ,A 组仅有10 % 的病人为优良。术后A、B两组均未见不良反应,C 组不良反应率达35 % 。结论:VB12 硬膜外注射可有效缓解前列腺摘除术后膀胱痉挛性疼痛且无不良反应。

Abstract:

Aim: To evaluate the effect of epidural drip of vitamin B 12 on relief of bladder spastic pain after prostatectomy. Methods: Sixty patients were divided into three groups randomly(n=20, in each group):A,B and C.Group A received epidual injection of normal saine (NS) 10ml. Group B had 1mg Vitamin B 12 , and group C 2mg morphine,both of them were diluted to 10ml with normal saline and given epidurally. They were injected 45 minutes after the final epidual infusion. Double blind method was observed to rate pain relief and the related complications for 72 hours postoperatively. Results: The effective rate of postoperative analgesia for group A,B and C were 10%, 85% and 95% respectively. The rate of side effects was 0%, 0% and 35% respectively. Conclusion: Vitamin B 12 is useful to prevent bladder spastic pain with little side effects.
