Abstract:cases of primary trigeminal neuralgia, who had received a long term treatment without success, have been treated with explorative operation of cerebellopontine angle through retroauricular small incision under local anesthesia. According to the finding in the operation, microvascular decompression or resection of tumor were made. In 1210 cases a causal factor was revealed including blood vessel compression, tumor compression, compression of arachnoid cysts and adherence, only 10 cases remained unclear etiology. In 96.7% of cases the neuralgia disappeared immediately after operation. The pain was alleviated in 3%. So the short term effective rate was 99.6%. The method of treatment, type of operation, prophylaxis and treatment for complications are discussed.
